Family routines and interactions combined with in-home support of Pathways team help to keep the family both physically and emotionally intact. This year, 120 homeless families with children will build new lives with the support of the Pathways to Home program.

The UniHealth Board of Directors designated $1 million over two years towards recuperative care projects. Cedars-Sinai Community Benefit decided to support this work by granting UniHealth $250,000.

Pathways to Health + Home

Our Hospital at Home programs are designed for hospitals that require urgent in-home medical assessments of patients who have had an exacerbation of an existing disease or injury. Our Skilled Nursing Facility at Home programs are designed to offer patients a choice of receiving their post-acute medical rehabilitation in the safety and comfort of their own home. The proof is in our outcomes, driven by our specialty programs and highly trained staff. Our innovative model is perfect for home health patients because it adds the unique feature of a physician organization.

A Shared Care Plan that identifies the enrollee’s unique needs and a treatment must be created for each enrollee within the first month of being in the program. The Shared Care Plan must be updated regularly by program staff, with each service documented every month. For enrollees receiving housing services, a housing specialist will assess the enrollee within 30 days and continue to meet weekly during the first six months of program enrollment.

Hospital at Home

Oftentimes these individuals do not seek care until it is an emergency. Pathways will also accept referrals from other homeless navigator programs serving the City based on eligibility criteria. Pathways uses evidence-based outreach and care coordination practices to ensure good outcomes for individuals experiencing, or at-risk of experiencing, homelessness. Individuals are referred by specific organizations, engaged in real-time by outreach workers, and enrolled in the program if eligible. Services are referred following an assessment that determines each individual’s unique needs. An inter-disciplinary team comprised of an outreach worker, care coordinator, housing specialist, and a clinician works closely with the enrollee, providing ongoing connection to stabilizing services and social supports.

A group of thought leaders including experts, practitioners, state leaders, and researchers across sectors provide strategic input, expertise, and policy advice to the initiative. For more than a decade, UniHealth Foundation has invested in programs that support people experiencing homelessness as they progress from hospital discharge to permanent housing. A wide variety of services and supports are required along the way, and UniHealth has identified many interventions that help make patients’ journey healthy and successful. In addition, Pathways To Health™ provides a variety of programs and services offered both in-house and on an outreach basis.

Pathways To Health is a community-based holistic treatment and education center designed to provide physical and psychospiritual services which integrate traditional and alternative forms of healing. Pathways’ focus is the maintenance and promotion of optimum wellness by providing various programs and services. The center is staffed by a multidisciplinary team of New York State licensed and certified professionals, with referral services to a cadre of other health care providers.

The introduction and forward tells the story of what brought them together in 1995. Pathways was guided to a new location in Mt.Sinai, NY and converted a home on 3 acres as the healing center. More teachers were certified and ministers were ordained during that year.

Pathways Healers retreat was held in the beginning of October and Patricia taught what was to be her last class with all of the healers surrounding her in love. On December 10th, 1999 Patricia Madigan Stringham transitioned into the Light with her husband, children and William and Judith at her side. She never was able to come to the new location but her teachings and love are with the center and it’s mission always. Thank you to all of the brave and dedicated healthcare professionals providing care in the Bay Area and around the world during this unprecedented global health emergency.
